Dr. Robert Mock Announces His Retirement After 40 Years Serving The Woodlands Area

My Dear Patients,

It is with mixed emotions that I am announcing my retirement from medical practice, effective immediately.

It has been my joy and honor to help serve you all over the last forty years, starting next to the old Jamail’s grocery store in “The Wharf” (Grogan’s Mill Shopping Center) and then moving to “The Woodlands Professional Building” here at 1120 Medical Plaza Drive, back when the idea of building even a single hospital in The Woodlands was just a dream. When my wife Cherry and I first started this practice, with her practicing alongside me in marriage, family, and child therapy, and with our baby son playing in the back office, it was our goal to provide a holistic approach to traditional medicine and psychotherapy for the community.

Though my wife’s early health problems prematurely forced her to leave her therapy practice, I was blessed by her tremendous support of my continued work to provide quality, compassionate medical care on my own. And, I have been doubly blessed by all of you, in you sharing with me the triumphs and tragedies of your own lives and sharing your trust in me as your family physician.

I had hoped to provide you a gradual transition for patient care when I eventually entered retirement, but 2020 has clearly had other plans for all of us. With the ongoing stresses of COVID-19 on my family, my practice, and myself, I have decided that I must promptly transition my patients’ care to other providers more equipped to handle the future challenges of this world to come.

Affiliates of Family Medicine, PA, the practice of Dr. Elena Garcia and Dr. Leticia Garcia-Seay, and their associate Dr. Aprill Rambarran, has graciously offered to provide you with continuing medical care, subject to your authorization for them to do so. All three physicians are well-trained through the University of Texas Medical School system and their practice has served the local community for over twenty years. They offer two locations in the area, one next to HCA Conroe Hospital, and one just east of I-45 off the Grand Parkway, with laboratory services available at both locations.

If you prefer, you may contact your medical plan/health insurance company for a list of area physicians. You may also contact the local medical society–or the physician referral service at a hospital of your choice—for names of physicians near you.

If you seek care elsewhere, I will forward a copy of your medical record to your new physician —when I receive your written authorization. Use the linked one, single-sided form titled “MEDICAL RECORDS RELEASE FORM” if you wish to continue care with Affiliates of Family Medicine and wish to release your records for them to use in your ongoing care. Use the separate, linked, two-sided form titled “AUTHORIZATION TO USE OR DISCLOSE PROTECTED HEALTH INFORMATION” if you wish to have your records sent to another physician or if you wish to request of a copy of your records, to be sent to your attention. My office will be able to send out records for the next three months; afterwards, Affiliates of Family Medicine will have your records available and handle records requests.

Your records from under my care will remain confidential with Affiliates of Family Medicine serving as new custodian of my records. If there are any outstanding lab or imaging test results that you have not reviewed with me in the office, by phone, or via my mailed results interpretation letters, please make an appointment promptly with Affiliates of Family Medicine or another healthcare provider and review those results at your visit. If you have any outstanding orders for labs or other testing, these have had to be cancelled and will need to be re-issued, as appropriate, at your visit with your new healthcare provider. Similarly, all requests for medication refills (even partial fills) and requests for insurance authorizations for specialist care aka “referrals” will need to be addressed by your new provider once you establish care with an office visit, or directly addressed with your pharmacist or insurance company.
