Patient Balances:
Payment is due when services are rendered. Please be prepared to pay any copay/deductible or other balances at your appointment.
Balances may be paid by credit/debit card (Visa, Mastercard, American Express, Discover), check, or cash.
In some cases, we may file your insurance claim first to allow your insurance plan to determine your exact financial responsibility; in these cases, payment will be due when your insurance claim processes or you receive a statement, whichever comes first.

Please check your insurance directory online or by calling your insurance to ensure that Dr. Mock is an in-network physician on your particular plan.

If you have a plan that requires you to select a primary care physician (PCP or PCCM), Dr. Mock must be effective with your insurance as your PCP for your insurance coverage to be valid.
Please note that Dr. Mock is currently NOT accepting any new Medicaid plans (except Medicaid QMB when secondary to Medicare).

If you don't have insurance, you are eligible for our self-pay discounted rates, both for office visits & labwork. These discounted rates are based on the Medicare Fee Schedule, to ensure you receive a fair, reasonable price, even when you're not paying for insurance, while still allowing us to stay in business and continue providing you with quality care. Office visits typically cost from $69 to $136, depending on the level of visit (i.e., how detailed/long/in depth it is). Labwork pricing depends on which tests must be performed and typically may run from $50 to $300.

You may qualify for federally and state-subsidized assistance for food, housing, childcare, medical care, and other needs. Local charities also may be able to provide assistance. If you are having difficulty paying for medications or medical testing (labwork, mammograms, etc.), please let us know. You may qualify for free or low-cost medicine through a patient assistance program (visit www.needymeds.org for details). Dr. Mock may be able to find a lower-cost medication or other treatment regime to address your medical needs. Various charities also offer free or subsidized mammograms and other medical testing. You have options to get the quality medical care you need. Just let us know and we will gladly help.
